General Requirements to Participate in Any Sport

MUST have an un-weighted 2.0 cumulative grade point average

2.0 cumulative GPA for Academic Grades and 2.0 cumulative GPA for Conduct Grades

Signed "FHSAA Consent and Release from Liability Certificate" (Included with the Athletic Physical Form)

May be completed at the free school sports physical OR be completed and signed by your doctor

Must be signed by your parent or guardian

Must be Notarized (Form must be notarized BEFORE comming to any of the Free School Physicals)

School Board Approved Insurance Policy is required for all sports

Basic school insurance required for all sports EXCEPT football

Board approved football insurance for football only

Show documentation for Proof of Age. Birth Certificate is preferred
Passport from a FOREIGN country - ONLY acceptable for athletes born OUTSIDE the United States
